	Order by E-Mail: jd@jd-art.com Send me a note indicating the items you are interested in. I will contact you personally by phone, email, or mail. I do my best to answer e-mail order inquiries within 24 hours!
	Order by Regular Mail: Complete and print the order form indicating the items you wish to purchase. Enclose the printed form with your Cashier's Check, Money Order, or Personal Check for the total amount due. Or, include your MasterCard or VISA credit card number and sign. Mail to JD ART, 2005 Tower Street, Trenton, MO 64683.(If you wish to pay by personal check, the item(s) will be shipped as soon as the check has been processed.)
	Order by FAX: Complete and print the order form indicating the items you wish to purchase. Add your credit card information and signature. FAX to 1-660-359-5970. FAX available 24 hours a day, 7 days a week.

SHIPPING INFORMATION

JD ART tries to ship within three (3) business days, but please allow up to two (2) weeks for delivery unless you need to meet a deadline; then, JD ART will try to meet your needs.

Items are shipped via U.S. Priority Mail unless otherwise requested.
